  • Imaging revealed multiple cerebral infarcts and a uterine fibroid with an abscess.
  • Infectious workup confirmed Streptococcus agalactiae bacteremia and abscess positivity.

Findings:

  • CT-guided drainage of the abscess yielded S. agalactiae.
  • Despite initial treatment, persistent fever and leukocytosis necessitated a total abdominal hysterectomy.
  • Intraoperative findings confirmed a necrotic, purulent uterine fibroid as the source of infection.

Implications:

  • Pyomyoma should be considered in postmenopausal or postpartum patients with vascular disease presenting with the diagnostic triad.
  • Delayed diagnosis of pyomyoma can lead to overwhelming sepsis and increased mortality.
  • Prompt diagnosis and treatment, including surgical intervention when necessary, are vital for improved patient outcomes.
