Diabetes: Management and Pharmacotherapy01:15

Diabetes: Management and Pharmacotherapy

216
The therapy for diabetes aims to alleviate hyperglycemia-related symptoms, prevent acute metabolic decompensation, and reduce chronic end-organ complications. Glycemic control is evaluated through short-term (self-monitoring, continuous glucose monitoring) and long-term (A1c, fructosamine) metrics, enabling near real-time tracking of blood glucose levels and reflecting glycemic control over specific time frames.
Insulin remains the cornerstone of treatment for most patients with type 1 and many...

Diabetes Mellitus: Type 2 and Gestational01:22

Diabetes Mellitus: Type 2 and Gestational

2.1K
Type 2 diabetes, characterized by insulin resistance, arises when the insulin receptors on cells lose responsiveness to insulin, diminishing the cell's capacity to take up glucose, resulting in elevated blood glucose levels. To receive a diagnosis of Type 2 diabetes, a series of blood glucose tests are necessary to assess whether the blood glucose falls within normal parameters. If the result is out of the normal range, a patient may be diagnosed as prediabetic or diabetic, depending on the...

Diabetes Mellitus: Overview and Type I Subtype

2.3K
Diabetes mellitus is a chronic metabolic disorder characterized by high blood glucose levels due to inadequate insulin production, insulin resistance, or both. The condition affects millions worldwide and can significantly impact their health and quality of life.
Type 1 diabetes is an autoimmune disease in which the immune system mistakenly attacks and destroys the insulin-producing beta cells in the pancreas. As a result, the body is unable to produce sufficient insulin, and individuals with...

Patient-centered Care01:13

Patient-centered Care

1.9K
Patient-centered care involves delivering care beyond inpatient hospitalization. Reflective practice can enhance a patient-centered approach. Reflective practice is a process of reasoning that considers all aspects of the present situation, including practicalities, learning from personal practice, and consideration of patient needs. Patients appreciate care decisions made while considering their input. Involving the patient in their care provides the patient with a sense of contribution rather...

Summary

Most endocrinology centers document gender identity and pronouns for type 1 diabetes patients, but sexual orientation documentation is less common. Improving sexual orientation and gender identity data collection is crucial for LGBTQ+ individuals with diabetes.

Area of Science:

  • Endocrinology
  • Public Health
  • Health Equity

Background:

  • Type 1 diabetes management presents unique challenges for LGBTQ+ individuals.
  • The Health Resources and Services Administration mandates sexual orientation and gender identity (SOGI) data collection.
  • Understanding SOGI documentation practices is vital for providing inclusive care.

Purpose of the Study:

  • To assess the current state of SOGI data documentation in the T1D Exchange Quality Improvement Collaborative.
  • To evaluate LGBTQ+ supportive and inclusive care practices in endocrinology centers.

Main Methods:

  • A survey was administered to 33 pediatric and 21 adult endocrinology centers.
  • The survey queried electronic health record documentation of SOGI, including gender identity, pronouns, sexual orientation, and sex assigned at birth (SAAB).
  • Fisher exact tests analyzed response frequencies and differences between pediatric and adult centers.

Main Results:

  • 64% of pediatric centers and 50% of adult centers had strategies for SOGI documentation.
  • Pediatric centers showed higher rates of documenting gender identity (P=0.04) and pronouns (P=0.02) compared to adult centers.
  • Documentation of sexual orientation was less consistent across centers.

Conclusions:

  • While gender identity and pronoun documentation are improving, sexual orientation documentation requires enhancement.
  • Further research is needed to optimize SOGI documentation and its impact on LGBTQ+ patients with type 1 diabetes.
  • Inclusive SOGI data collection is essential for equitable diabetes care.
