    Area of Science:

    • Mycology
    • Infectious Diseases
    • Ophthalmology

    Background:

    • Zygomycosis is a rare but aggressive fungal infection.
    • Diabetes mellitus is a significant risk factor for invasive fungal infections.
    • Orbital and intracranial involvement indicates a severe, life-threatening presentation.

    Observation:

    • A 63-year-old male with diabetes presented with acute sinusitis.
    • Symptoms rapidly progressed over one week to include orbital and intracranial signs.
    • The patient developed extensive fungal infiltration and mycotic thrombosis.

    Findings:

    • Despite diabetes control, surgical debridement, and antifungal therapy, the patient died.
    • Pathological examination revealed extensive fungal infiltration of periorbital tissues.
    • Mycotic thrombosis led to necrosis and infarction of orbital structures and extraocular muscles.

    Implications:

    • Zygomycosis requires prompt diagnosis and aggressive management, especially in diabetic patients.
    • Early surgical intervention and antifungal therapy are crucial but may not always prevent fatal outcomes.
    • This case highlights the devastating potential of zygomycosis, emphasizing the need for heightened clinical suspicion and rapid treatment protocols.
