    Background:

    • Drug intoxication, particularly from lipid-soluble organic molecules like glutethimide and barbiturates, presents a significant clinical challenge.
    • Hemoperfusion is an extracorporeal technique used to remove toxins from the blood.
    • Amberlite XAD-4 resin is a macroreticular adsorbent with a known affinity for organic molecules.

    Observation:

    • Eight patients with drug intoxication underwent hemoperfusion using a 650gm Amberlite XAD-4 resin column.
    • Blood flow rate was maintained at 300 ml/min.
    • Treatment sessions varied in duration from 2.5 to 10 hours.

    Findings:

    • Column clearances for glutethimide and barbiturates ranged from 207 to 300 ml/min.
    • One patient recovered after removing over 30 gm of glutethimide across three days of hemoperfusion.
    • Patients showed dramatic clinical responses without significant toxic reactions.

    Implications:

    • Amberlite XAD-4 resin hemoperfusion is a simpler and more effective method for removing barbiturates and glutethimide compared to existing treatments.
    • This technique offers a promising therapeutic option for managing severe drug overdoses.
    • Further research into resin-based hemoperfusion for various intoxicants is warranted.
