Summary

Local antibiotic use, not prophylactic antibiotics, independently predicted fracture-related infection (FRI) in open tibia fractures. Optimizing treatment protocols for open fractures is crucial for reducing infection rates.

Background:

Prophylactic antibiotics are standard for open fractures to prevent fracture-related infection (FRI).
Despite routine use, FRI rates in Gustilo Anderson (GA) type III open fractures remain high.
This study examined FRI rates and prophylactic antibiotic trends in open tibia fractures.

Purpose of the Study:

To investigate the incidence of fracture-related infection (FRI) in type III open tibia fractures.
To analyze trends in prophylactic and local antibiotic usage over a 15-year period.
To identify independent predictors of FRI in open tibia fractures.

Main Methods:

Retrospective analysis of 133 type III open tibia fractures (2007-2021).
Data collected on prophylactic antibiotics, local antibiotics, soft tissue coverage, and FRI.
Statistical analysis included chi-squared tests and multivariable logistic regression.

Main Results:

Fracture-related infection (FRI) occurred in 24.8% of cases.
Trends showed increased use of 3rd generation cephalosporins and decreased use of 1st/2nd generation.
Local antibiotic use was the sole independent predictor of FRI (OR 9.3, p < 0.001).

Conclusions:

Local antibiotic administration is a critical factor in preventing FRI.
Institutional protocols and education are needed to improve recognition and treatment of high-risk open fractures.
Further research into optimal antibiotic strategies for open fractures is warranted.
