Myotendinous Rupture of the Posterior Tibial Tendon with Associated Fibular Fracture: A Case Report

Case:
A 20-year-old man presented after a fall from an 8-foot ladder. Radiographs revealed pronation external rotation injury. Closed reduction and splinting was performed in the emergency department. Surgery was planned 6 days later to include open reduction and internal fixation of the fibula and syndesmosis, and open repair of deltoid ligament. Intraoperatively, a myotendinous rupture of the posterior tibial tendon (PTT) was identified and repaired.
Conclusion:
PTT ruptures are rarely reported but should be suspected in high-energy mechanism ankle fractures in all patients. Direct repair can result in excellent outcomes.
