- Device interrogation showed increased right ventricular (RV) pacing, necessitating a device upgrade.

- An attempted ICD upgrade revealed a significant Superior Vena Cava (SVC) obstruction.
- This obstruction precluded standard transvenous lead placement.

- A leadless pacemaker was successfully implanted in the right atrium via a femoral venous approach.
