Successful Leadless Pacemaker Implantation Despite SVC Obstruction

• Device interrogation showed increased right ventricular (RV) pacing, necessitating a device upgrade.

• An attempted ICD upgrade revealed a significant Superior Vena Cava (SVC) obstruction.
• This obstruction precluded standard transvenous lead placement.

Findings:

• A leadless pacemaker was successfully implanted in the right atrium via a femoral venous approach.
• This demonstrated a viable alternative in the presence of SVC obstruction.

Implications:

• Permanent leadless pacemakers offer a safe and less invasive alternative for complex cases.
