Clinical Trials: Overview

3.4K
Clinical development focuses on how the drug will interact with the human body and encompasses four key phases of clinical trials, each serving a specific purpose in assessing the safety and effectiveness of new drugs. These phases overlap and build upon one another. Phase I involves a small group of healthy volunteers (typically 20-80 individuals) or, in cases where significant toxicity is expected, patients with the targeted disease, such as cancer or AIDS. The volunteers are tested for...

Clinical Trials

9.3K
Clinical trials are prospective experimental studies conducted on humans to determine the safety and efficacy of treatments, drugs, diet methods, and medical devices. Using statistics in clinical trials enables researchers to derive reasonable and accurate conclusions from the collected data, allowing them to make wise decisions in uncertain situations. In medical research, statistical methods are crucial for preventing errors and bias.
There are four phases in a clinical trial. A phase one...

Continuous Renal Replacement Therapy01:30

Continuous Renal Replacement Therapy

156
Continuous Renal Replacement Therapy, also known as CRRT, is a procedural treatment for acute kidney injury (AKI) that gradually removes uremic toxins and fluids while maintaining acid-base balance and stabilizing electrolytes. It is particularly useful for hemodynamically unstable patients. Unlike intermittent hemodialysis, which is faster, CRRT provides a gentler approach over 24 hours, closely mimicking the function of natural kidneys. However, CRRT is not ideal for patients with...

Obesity01:24

Obesity

631
The Body Mass Index (BMI) is a numerical value derived from a person's weight and height, used to categorize individuals into weight ranges. It is calculated using the formula: weight in kilograms divided by height in meters squared. Obesity is a health condition characterized by excessive accumulation of adipose tissue that poses health risks, often diagnosed with a BMI ≥ 30. This excess fat storage occurs when surplus dietary calories are converted into triglycerides and stored in...

Diabetes: Management and Pharmacotherapy

412
The therapy for diabetes aims to alleviate hyperglycemia-related symptoms, prevent acute metabolic decompensation, and reduce chronic end-organ complications. Glycemic control is evaluated through short-term (self-monitoring, continuous glucose monitoring) and long-term (A1c, fructosamine) metrics, enabling near real-time tracking of blood glucose levels and reflecting glycemic control over specific time frames.
Insulin remains the cornerstone of treatment for most patients with type 1 and many...

Summary

A patient-delivered lifestyle intervention for weight loss maintenance (WLM) showed better results than standard professional care. This approach, using mentors and peer support, offers a sustainable model for obesity treatment.

Area of Science:

  • Obesity research
  • Behavioral medicine
  • Public health

Background:

  • Weight loss maintenance (WLM) is a significant challenge in obesity treatment.
  • Current continuous care models are effective but costly and unsustainable.
  • Novel, cost-effective WLM strategies are needed.

Purpose of the Study:

  • To compare the efficacy of an entirely patient-delivered WLM treatment versus professionally delivered standard-of-care (SOC).
  • To evaluate a novel WLM intervention model for sustainability and effectiveness.

Main Methods:

  • A 2-phase randomized clinical trial was conducted.
  • Phase 1 involved an online weight loss program; eligible participants entered Phase 2 (18-month WLM trial).
  • Phase 2 randomized participants to either patient-delivered (mentors and peers) or professional SOC WLM interventions.

Main Results:

  • Patient-delivered WLM resulted in significantly less weight regain compared to SOC at 18 months (0.77 kg vs 2.37 kg).
  • Significant improvements were also observed in diastolic blood pressure, heart rate, physical activity, and sedentary behavior with the patient-delivered intervention.
  • The patient-delivered intervention demonstrated superior outcomes in weight management and cardiovascular risk factors.

Conclusions:

  • Patient-delivered lifestyle interventions, utilizing mentors and peer support, are more effective for WLM than professional SOC.
  • This novel approach offers a promising, potentially more sustainable model for long-term obesity management.
  • Further research should explore the implementation of this model in diverse community and clinical settings.
