Shenrong Guben Huanshao Pill (SRGBHSP) is a traditional Chinese medicine (TCM) compound preparation derived from ancient classical prescriptions, widely utilized in clinical treatment of chronic kidney injury, neurasthenia, osteoporosis, and other symptoms associated with kidney deficiency syndrome. The functions of brain and marrow are believed to be crucially dependent on sufficient kidney-yang in TCM theory. Concurrently, modern studies have demonstrated that kidney tonic drugs possess pharmacological effects, including antioxidant, anti-inflammatory as well as regulating neurotransmitter levels. Therefore, we hypothesize that SRGBHSP may exert neuroprotective effect indirectly or directly by regulating kidney function.

This study employed network pharmacology to investigate the potential targets and mechanisms underlying the neuroprotective effects of SRGBHSP. Subsequently, OGD/R and intraperitoneal injection of LPS were utilized to establish a model of nerve cell injury and neuroinflammation in rats, allowing for an in-depth investigation of the neuroprotective effects of SRGBHSP. Finally, signature metabolites and metabolic pathways were analyzed through multi-omics approaches, including 16S rRNA analysis of intestinal flora and metabolomics.

Network pharmacological analysis identified 184 potential targets associated with the neuroprotective effect of SRGBHSP. Pharmacodynamic studies demonstrated that SRGBHSP can inhibit apoptosis, enhance MMP, decrease Ca²⁺ accumulation and alleviate cellular oxidative stress. Additionally, SRGBHSP improved behavioral performance, histopathological changes, monoamine neurotransmitter levels; Western blotting results indicated that SRGBHSP may exert its neuroprotective effect by regulating Wnt/β-catenin signaling pathway, thereby reducing neuronal apoptosis and inhibiting inflammatory response. Further multi-omics analysis indicated that the neuroprotective effect of SRGBHSP may also be associated with the modulation of metabolic pathways, such as the GABAergic synapse.

This study integrated network pharmacology, in vivo and in vitro experiments, and multi-omics analysis to demonstrate for the first time the neuroprotective effect of SRGBHSP. It also reveals a potential mechanism involving the regulation of Wnt/β-catenin signaling pathway and the "microbe-gut-brain" axis, contributing to a protective effect on the nervous system.
