Summary

Platelet-rich plasma (PRP) injections significantly improved tennis elbow functional outcomes compared to corticosteroid and saline treatments. While PRP showed superior results in pain and function scores, imaging measures did not correlate with clinical improvements.

Background:

  • Lateral epicondylitis (tennis elbow) is a common condition causing elbow pain.
  • Current treatments like corticosteroid, platelet-rich plasma (PRP), and saline injections have unclear comparative efficacy.
  • This study aimed to compare the effectiveness of these injection therapies.

Purpose of the Study:

  • To compare the efficacy of platelet-rich plasma (PRP) versus corticosteroid and saline injections for treating lateral epicondylitis.
  • To evaluate functional and clinical outcomes using validated scoring systems and radiological findings.
  • To test the hypothesis that PRP yields superior results.

Main Methods:

  • Fifty-five patients with chronic lateral epicondylitis were randomized into three groups: glucocorticoid, PRP, and saline injections.
  • Functional outcomes were assessed using Visual Analog Scale (VAS), Patient-Rated Tennis Elbow Evaluation (PRTEE), and Disabilities of the Arm, Shoulder, and Hand (DASH) scores.
  • Radiological evaluations included ultrasonography for vascularity and superb microvascular imaging (SMI) at baseline and three months.

Main Results:

  • Thirty-six patients completed the study; PRP and glucocorticoid groups showed no significant difference in VAS scores at three months.
  • PRP treatment resulted in significantly better DASH and PRTEE scores compared to both glucocorticoid and saline groups (p < 0.001).
  • Radiological assessments of vascularity and SMI did not show significant differences between groups and did not correlate with functional outcomes.

Conclusions:

  • Platelet-rich plasma (PRP) injections demonstrate superior functional outcomes for lateral epicondylitis in early to mid-term follow-up compared to glucocorticoid and saline.
  • Ultrasonographic measures of vascularity and superb microvascular imaging (SMI) do not appear to reliably predict clinical outcomes in this condition.
  • PRP represents a promising therapeutic option for lateral epicondylitis, offering enhanced functional recovery.
