    Summary

    Cholesterol emboli syndrome, a complication of advanced atherosclerotic vascular disease, presents with skin and organ damage. Diagnosis is confirmed by identifying cholesterol clefts in arterial biopsies.

    • Three patients with advanced atherosclerosis presented with livedo reticularis and purple toes.
    • Distal necrosis, abdominal pain, and renal function changes were noted despite palpable pulses.

    Findings:

    • Cutaneous biopsy revealed characteristic cholesterol clefts within atheromatous debris in small arteries.
    • These findings confirmed the diagnosis of cholesterol emboli syndrome.

    Implications:

    • Understanding CES pathophysiology is crucial for managing patients with atherosclerotic disease.
    • Early recognition and diagnosis can potentially improve patient outcomes.
