Summary

In chronic meniscal injuries, elevated synovial fluid inflammation markers at surgery correlate with worse long-term outcomes. Acute injuries show high inflammation but no significant impact on long-term results.

Background:

  • Synovial fluid (SF) biomarkers vary over time post-acute knee injury.
  • Persistently high inflammatory markers may predict poorer long-term outcomes.

Purpose of the Study:

  • To examine the link between SF biomarkers at meniscectomy and long-term patient outcomes.
  • To compare outcomes in acute versus chronic meniscal injuries based on SF biomarkers.

Main Methods:

  • Retrospective cohort study (Level of evidence, 3) of 85 patients undergoing arthroscopic meniscectomy.
  • SF analyzed for 10 inflammatory biomarkers; patient-reported outcomes (PROs) assessed over minimum 4 years.
  • K-means clustering used to define high- and low-inflammation SF biomarker profiles.

Main Results:

  • Two clusters identified: high-inflammation (7 proinflammatory biomarkers elevated) and low-inflammation.
  • High-inflammation cluster predominantly linked to chronic injuries.
  • In chronic injuries, high-inflammation SF profile correlated with worse pain, function (Lysholm, KOOS-PS), activity (Tegner), and increased injections post-surgery.

Conclusions:

  • A proinflammatory SF biomarker profile in chronic meniscal injury predicts worse surgical outcomes.
  • Acute meniscal injuries typically present with high SF inflammation, but this does not negatively impact long-term results.
