Summary

The incidence of endophthalmitis after retina surgery is 0.28%, with higher risks in older patients and those with complex conditions. Surgeon factors did not influence endophthalmitis risk.

Background:

Postoperative endophthalmitis is a serious complication following intraocular surgery.
Understanding risk factors is crucial for prevention and improving patient outcomes.

Purpose of the Study:

To determine the incidence of endophthalmitis in Medicare beneficiaries undergoing retina surgery.
To identify patient and surgeon-related risk factors for postoperative endophthalmitis.

Main Methods:

Retrospective analysis of Medicare beneficiaries from 2016-2022.
Exclusion criteria included prior endophthalmitis, recent intraocular procedures, or injections.
Multivariable logistic regression analyzed factors associated with endophthalmitis within 42 days of surgery.

Main Results:

Overall endophthalmitis incidence was 0.28% (1,019 cases out of 369,471 procedures).
Higher risks observed for patients aged ≥85, Charlson Comorbidity Index ≥3, macular surgery, and complex retinal detachment repair.
Concurrent cataract surgery was linked to a decreased risk.

Conclusions:

The incidence of endophthalmitis following retina surgery is low but significant.
Patient factors like age and comorbidities, along with surgical indication, are key risk factors.
No association was found between surgeon characteristics and endophthalmitis risk.
