Blinding

Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. A placebo effect occurs when untreated subjects falsely believe they have received the treatment and report improved symptoms.   • To systematically review and synthesize the literature on interventions designed to reduce implicit racial bias among GME trainees.
  • To evaluate the methodologies and outcomes of existing bias mitigation strategies in GME.
  • To identify gaps in research and inform future evidence-based intervention development.

Main Methods:

  • Comprehensive literature search of PubMed, Embase, and Academic Search Premier (2003-2024).
  • Screening and data extraction by two independent reviewers.
  • Methodological quality assessment using the Medical Education Research Study Quality Instrument.
  • Framework synthesis using transformative learning theory and Kirkpatrick Model.

Main Results:

  • 23 studies were eligible from 6455 records.
  • Common strategies included critical reflection (87%) and instructional activities (65%).

  • Two of three studies measuring direct knowledge/behavior (Kirkpatrick Level 2B) showed short-term bias reduction.

  • Current interventions often rely on self-reported outcomes, with limited evidence of direct, sustained behavioral change.
  • There is a critical need for rigorous research to develop and implement evidence-based interventions.
  • Future research should focus on interventions that demonstrably reduce the impact of implicit racial bias on patient-centered health outcomes.
