Two-Stage Re-revision for Tuberculosis Affecting a Revision Total Hip Arthroplasty: A Case Report

Case:
A 68-year-old woman presented with pain in her left hip and a discharging sinus for 8 months. Ten months previously, she had undergone a revision total hip arthroplasty (THA) for aseptic loosening of a THA initially performed 30 years ago. This time, she underwent a 2-stage re-revision. Targeted next-generation sequencing identified the organism as Mycobacterium Tuberculosis, followed by antitubercular therapy for 9 months. After a 5-year follow-up, Harris Hip Score was 85%, and there was no evidence of infection or prosthetic loosening.
Conclusion:
High index of suspicion and genomic sequencing may be required to identify the organism in tubercular periprosthetic infections.
