  • A multinational, population-based observational study analyzed five health system infrastructure elements: mammography, CT imaging, radiation oncology, surgery, and hospitals.
  • Data were collected and analyzed to benchmark infrastructure availability against international targets, prioritizing indicators with timely, comprehensive, consistent, standardized, and reliable data.

Main Results:

  • Commonwealth countries generally fall short of international targets for cancer control infrastructure, except for mammography diagnostics.
  • Substantial deficits exist in CT imaging, healthcare facilities, and surgery, with significant disparities observed across regions, income groups, and state sizes.
  • Radiation oncology showed the greatest inequities, with variations up to 62 times by country income group.

Conclusions:

  • Major infrastructure deficits in the Commonwealth can negatively impact the availability and quality of cancer screening, diagnosis, and treatment.
  • Recommendations include establishing a Cancer Control Data Observatory for standardized data collection, extending pooled procurement mechanisms, and fostering public-private partnerships for infrastructure expansion.
