Microtia/aural atresia reconstruction surgery focuses on improving hearing and cosmesis in patients with microtia, aural atresia, and aural stenosis. Preoperative evaluation of microtia with high-resolution CT plays an important role in assessing the potential for the successful surgical restoration of hearing. Given the challenges of the procedure, surgical candidates are carefully selected, taking into account the CT imaging findings. The Jahrsdoerfer scoring system is a semiquantitative tool used in the selection of suitable surgical candidates to predict the postoperative hearing outcome in patients with microtia and aural atresia spectrum based on anatomic features on noncontrast temporal bone CT. This pictorial guide illustrates the relevant anatomy and key imaging findings in the Jahrsdoerfer microtia grading system.
