    Diaphragmatic electromyography (EMG) spectral analysis shows changes during hyperpnea. However, these EMG changes are not specific enough to reliably predict ventilatory failure.

    Background:

    • Voluntary hyperpnea challenges respiratory muscles.
    • Electromyographic (EMG) spectral analysis can assess muscle fatigue and function.

    Purpose of the Study:

    • To investigate changes in diaphragmatic EMG spectral characteristics during voluntary hyperpnea.
    • To evaluate the utility of EMG spectral analysis in predicting ventilatory failure.

    Main Methods:

    • Recorded diaphragmatic EMG using surface and esophageal electrodes.
    • Calculated power spectral density (PSD) and characterized by H/L ratio and centroid frequency (Fc).
    • Analyzed EMG spectral changes during varying levels of voluntary hyperpnea.

    Main Results:

    • Significant decreases in H/L ratio and Fc observed during high levels of hyperpnea, consistent between electrode types.
    • EMG Fc changes detected during sustainable hyperpnea, unlike H/L ratio changes.
    • H/L ratio changes only significant during unsustainable hyperpneic loads, suggesting lower sensitivity.

    Conclusions:

    • Diaphragmatic EMG spectral analysis reveals changes during hyperpneic loads.
    • The centroid frequency (Fc) is more sensitive to sustainable hyperpnea than the H/L ratio.
    • The specificity of diaphragmatic EMG spectral changes for predicting ventilatory failure is questionable.
