Insomnia is a prevalent sleep disorder characterized by difficulty falling asleep, frequent awakenings during the night, and waking up too early without being able to return to sleep. People with insomnia often experience these disruptions at least three nights a week for at least one month. Summary

Post-traumatic stress disorder (PTSD) with insomnia shows distinct brain activity in emotion regulation areas compared to PTSD without insomnia. These neural differences may help identify biomarkers for PTSD and guide new treatments.

  • Functional magnetic resonance imaging (fMRI) was used to study 46 PTSD patients (23 with insomnia, 23 without) and 28 controls.
  • Participants completed the Shifted Attention Emotion Appraisal Task (SEAT) during fMRI scans.
  • Whole-brain and region-of-interest (ROI) analyses compared neural activity.

Main Results:

  • PTSD patients with insomnia showed reduced activation in the left dorsolateral prefrontal cortex (BA46) and left inferior frontal gyrus (BA47) during implicit emotion processing.
  • Deactivation in the left inferior temporal gyrus (BA37) was observed during appraisal-based emotion regulation.
  • Insomnia severity correlated with activation in the left fusiform gyrus (BA37) in the PTSD-insomnia group.

Conclusions:

  • Identified distinct neural mechanisms in PTSD patients with comorbid insomnia.
  • Altered brain activity in specific regions may serve as biomarkers for PTSD with insomnia.
  • These findings suggest potential targets for novel therapeutic interventions.
