Background:

  • Sarcopenia is linked to poorer outcomes and increased toxicity in head and neck squamous cell cancer (HNSCC).
  • Understanding sarcopenia's dynamic changes is crucial for predicting patient prognosis in operable HNSCC.

Purpose of the Study:

  • To evaluate sarcopenia trajectories in operable HNSCC patients.
  • To assess the association between these trajectories and overall survival (OS) and disease-free survival (DFS).

Main Methods:

  • Body composition was analyzed pre- and post-treatment in 332 patients with stage II-IV HPV-negative HNSCC undergoing surgery.
  • Sarcopenia trajectories were classified into four groups: remains sarcopenic, remains non-sarcopenic, recovers from sarcopenia, or progresses to sarcopenia.
  • Multiple Cox regression was used to analyze survival outcomes.

Main Results:

  • 108 patients were sarcopenic at baseline, with 106 remaining sarcopenic post-treatment.
  • Patients who remained sarcopenic or progressed to sarcopenia had significantly worse OS and DFS compared to those who remained non-sarcopenic.
  • Adjustments were made for confounding factors in the survival analysis.

Conclusions:

  • Sarcopenia trajectories serve as dynamic biomarkers in operable HNSCC.
  • These dynamic changes in sarcopenia can predict oncologic outcomes, including survival.
