    Background:

    • Chronic subdural hematoma (CSH) is a collection of blood between the dura mater and arachnoid mater.
    • Open heart surgery involves complex procedures with potential complications.
    • CSH following cardiac surgery is a rare but significant concern.

    Observation:

    • Three patients developed CSH after undergoing open heart surgery.
    • Computed tomography (CT) scans showed high-density subdural collections post-cardiac surgery.
    • These collections evolved to isodensity or low density with mass effect within 2-3 weeks.

    Findings:

    • Anticoagulant therapy (heparin) and rapid changes in intracranial volume (e.g., due to mannitol) are potential contributing factors.
    • Tearing of bridging veins may occur during these rapid volume shifts.
    • Liquefied hematoma (45-60 mL) was aspirated, and the hematoma cavity's outer membrane was identified via trepanation.

    Implications:

    • Early recognition and management of CSH are crucial in post-cardiac surgery patients.
    • Understanding the etiological factors can help in preventing CSH in this population.
    • Surgical intervention, such as aspiration and membrane recognition, can be effective in treating CSH.
