Background:

  • Lung adenocarcinoma (LDA) is a major cause of cancer mortality.
  • Understanding factors influencing prognosis in early-stage LDA is crucial for treatment planning.

Purpose of the Study:

  • To investigate the association between smoking extent and prognosis, imaging features, and clinicopathological factors in clinical stage I lung adenocarcinoma (c-stage I LDA).

Main Methods:

  • Analysis of 2,285 patients with c-stage I LDA who underwent surgical resection (2010-2018).
  • Classification into never, light (Brinkman Index [BI] 0-600), and heavy smokers (BI > 600).
  • Evaluation of clinicopathological features, imaging characteristics (solid proportion, SUVmax), and overall survival (OS).

Main Results:

  • Significant differences in OS were observed across smoking groups.
  • Heavy smokers showed more invasive imaging characteristics (larger solid proportion, higher SUVmax).
  • Heavy smoking (HR 2.071), older age (HR 1.111), and vascular invasion (HR 2.312) were independently associated with poorer OS.

Conclusions:

  • Smoking extent is an independent predictor of poorer survival, larger solid tumor size, and higher SUVmax in c-stage I LDA.
  • Age and vascular invasion are significant prognostic factors, especially in heavy smokers.
