    Summary

    A novel V1th heart sound, undetectable by auscultation, is described. This low-frequency systolic sound may be physiological in healthy individuals or pathological in aortic incompetence.

    Context:

    • A novel systolic sound, termed the V1th heart sound, has been identified.
    • This sound is not audible through traditional auscultation but is detectable via specific frequency analysis.

    Purpose:

    • To describe the characteristics and potential origins of the newly identified V1th heart sound.
    • To differentiate between physiological and pathological presentations of the V1th heart sound.

    Summary:

    • The V1th heart sound is a low-frequency (35 Hz, rarely 70 Hz) systolic event, maximal at the apex.
    • It presents as 1-2 oscillations in late-systole and can be found in healthy subjects (physiological, low amplitude, further from the 2nd heart sound) and patients with aortic incompetence (pathological, closer to the 2nd heart sound).
    • Two hypotheses regarding the origin of this sound are proposed.
