Summary

Fuchs endothelial corneal dystrophy (FECD) shows regional guttae variations, with the superior cornea being relatively spared. This suggests potential ultraviolet (UV) protection benefits, indicating UV avoidance may help manage FECD progression.

Area of Science:

  • Ophthalmology
  • Corneal Diseases
  • Cell Biology

Background:

  • Fuchs endothelial corneal dystrophy (FECD) is a progressive condition affecting the cornea.
  • Guttae distribution patterns in FECD are not fully understood.
  • The role of environmental factors, such as ultraviolet (UV) radiation, in FECD progression is under investigation.

Purpose of the Study:

  • To investigate regional variations in guttae distribution within the cornea in patients with FECD.
  • To evaluate the relationship between guttae distribution and anatomical ultraviolet (UV) protection provided by the eyelid.

Main Methods:

  • A prospective observational study was conducted involving 19 eyes of 19 FECD patients.
  • Slit-scanning wide-field contact specular microscopy was used to analyze guttae distribution in five corneal regions.
  • Marginal reflex distance (MRD) was measured to assess eyelid position and potential UV protection.

Main Results:

  • The superior cornea exhibited a significantly lower proportion of guttae (29.4%) compared to central (69.0%), temporal (57.4%), and nasal (57.5%) regions (P < 0.05).
  • No significant correlation was found between marginal reflex distance (MRD) measurements and superior guttae distribution (P = 0.40).

Conclusions:

  • FECD progression demonstrates regional variation, with relative sparing in the superior cornea.
  • These findings suggest potential UV-protective mechanisms influencing FECD, even if not directly correlated with eyelid position.
  • UV protection may be a beneficial preventive strategy for early-stage FECD.
