Summary

Pain relief from diclofenac-hyaluronate (DF-HA) injections significantly improved motor function in advanced knee osteoarthritis (KOA) patients. However, balance improvements suggest additional mechanisms beyond pain reduction in refractory KOA.

Purpose of the Study:

  • To investigate the impact of pain relief following DF-HA injection on motor function in advanced KOA patients.
  • To assess changes in knee function, range of motion, muscle strength, and balance after DF-HA treatment.
  • To explore the relationship between pain reduction and motor function recovery in refractory KOA.

Main Methods:

  • Retrospective study of 100 knees (82 patients) with Kellgren-Lawrence grade III/IV KOA treated with DF-HA.
  • Patients divided into Improved (IM-G) and Non-Improved (Non-IM-G) groups based on VAS pain score changes 3-7 days post-injection.
  • Assessed Japanese Orthopaedic Association Score for Osteoarthritis of the Knee (JOAS), range of motion (ROM), quadriceps femoris strength (QF), and single-legged stance (SLS) pre- and post-injection.

  • The Non-Improved Group (n=12) demonstrated no significant changes in JOAS, ROM, or QF.
  • The Non-Improved Group showed a significant improvement in single-legged stance (SLS) (p=0.024), suggesting balance benefits beyond pain relief.

Conclusions:

  • Aggressive pain management with DF-HA significantly enhances key motor functions in advanced KOA patients who achieve pain relief.
  • Improvements in balance may involve mechanisms independent of pain reduction.
  • Effective pain control is crucial for improving motor function in refractory KOA.
