Background:

  • Advanced radiotherapy (RT) techniques improve prostate cancer (PCa) survival but often cause significant gastrointestinal (GI) and genitourinary (GU) toxicities.
  • Accurate prediction of these toxicities is essential for developing mitigation and preventative strategies to preserve patient quality of life.

Purpose of the Study:

  • To comprehensively review and summarize reported predictors of acute and late GI and GU toxicities in PCa patients undergoing conventional (CFRT), moderate (MHRT), and ultra-hypofractionation (UHRT).

Main Methods:

  • A scoping review of 169 studies published between 2000 and 2024 from four databases.
  • Analysis of studies investigating GI (127) and GU (78) toxicities, examining univariate, multivariate, and validation methodologies.
  • Categorization of identified predictors including dosimetric, patient, clinical, treatment, disease, genetic, and radiomic features.

Main Results:

  • Dosimetric predictors were most frequently reported, followed by patient, clinical, treatment, disease, genetic, and radiomic features.
  • Limited external validation, particularly multicenter studies, hinders the proven applicability and performance of identified predictors.
  • Most studies employed univariate analysis (139), with fewer using multivariate analysis (94) or internal (40) and external (5) model validation.

Conclusions:

  • While dosimetric factors are commonly identified, robust external validation is lacking for most predictors of RT-induced GI/GU toxicities in PCa.
  • Future research should integrate multimodality data, including radiomics, and prioritize prospective, multicenter external validation.
  • Standardized reporting and further investigation into delivered doses and sub-volumes are necessary to enhance reproducibility and comparability.
