    Summary

    Psoas abscess is a rare complication of inflammatory bowel disease (IBD), affecting only 0.6% of IBD patients in this study. Ulcerative colitis with psoas abscess was notably unreported before this case series.

    Background:

    • Inflammatory bowel disease (IBD), encompassing Crohn's disease and ulcerative colitis, can present with extraintestinal manifestations.
    • Psoas abscess, a collection of pus in the iliopsoas muscle, is a known but infrequent complication of various conditions.

    Observation:

    • This study reviewed cases of psoas abscess at an institution between 1979-1984.
    • Three cases of IBD patients with psoas abscess were identified: two with Crohn's ileocolitis and one with ulcerative colitis.
    • During the same period, 766 IBD patients were treated, yielding an incidence of 0.6% for psoas abscess complication.

    Findings:

    • Psoas abscess was a rare complication of IBD in the observed patient cohort.
    • IBD was not identified as a primary causative factor in the majority of psoas abscess cases reviewed.
    • This series includes the first reported case of ulcerative colitis complicated by psoas abscess.

    Implications:

    • The findings highlight the infrequent association between IBD and psoas abscess.
    • The report expands the known spectrum of IBD complications, particularly concerning ulcerative colitis.
    • Further research may be warranted to elucidate the specific mechanisms and risk factors for psoas abscess in IBD patients.
