    Background:

    • Surgical exploration of the common bile duct (CBD) for gallstones often results in a high rate of residual stones.
    • Conventional CBD exploration techniques are performed blindly, lacking direct visualization.
    • There is a lack of reliable methods for post-exploration checks before duct closure, typically involving T-tube drainage.

    Purpose of the Study:

    • To evaluate the efficacy of operative choledochoscopy in reducing residual common bile duct stones.
    • To assess the safety and benefits of using choledochoscopy for intraoperative visualization and stone extraction.
    • To determine the feasibility of primary CBD closure without T-tube drainage following choledochoscopy.

    Main Methods:

    • A series of 150 patients underwent operative choledochoscopy using a flexible fiberoptic choledochoscope.
