Nephrotic Syndrome II : Assessment and Medical Management

26
IntroductionNephrotic syndrome is a kidney disorder marked by excessive protein loss in the urine, leading to various systemic complications. This condition often results from damage to the glomeruli—the kidney's filtering units—causing proteinuria, low blood protein levels, and fluid retention. Understanding the assessment, diagnosis, and management of nephrotic syndrome is essential for effective treatment and prevention of further kidney damage.AssessmentPatient History: Document...

Area of Science:

  • Ophthalmology
  • Immunology
  • Pediatrics

Background:

  • Scleritis, an inflammatory scleral condition, can lead to severe vision loss and is often linked to systemic diseases.
  • Non-infectious scleritis in children is rarely reported, posing diagnostic and therapeutic challenges.
  • Conventional treatments like glucocorticoids may be effective but often result in disease recurrence.

Observation:

  • A pediatric case of non-infectious scleritis refractory to glucocorticoid therapy is detailed.
  • Extensive investigations excluded infectious etiologies for the scleritis.
  • The patient experienced disease recurrence during attempts to reduce glucocorticoid dosage.

Findings:

  • Sequential administration of tocilizumab followed by tofacitinib achieved successful disease remission.
  • The patient was weaned off glucocorticoids and all immunosuppressive therapy.
  • Complete drug cessation was maintained for two years post-treatment.

Implications:

  • This case highlights tocilizumab and tofacitinib as a potential effective treatment strategy for pediatric non-infectious scleritis.
  • The findings suggest a new diagnostic and treatment paradigm for clinicians managing this condition.
  • This approach may offer an alternative for patients unresponsive to or intolerant of conventional therapies.
