Mini-open partial fasciotomy is effective for Dupuytren contracture, with recurrence in 36% of joints within 3.7 years. Younger age and male sex are linked to higher recurrence rates after this procedure.

Area of Science:

  • Orthopedics
  • Hand Surgery
  • Plastic Surgery

Background:

  • Dupuytren contracture recurrence is common in younger patients.
  • Mini-open fasciotomy preserves tissue at the palmar digital crease.
  • This technique aims to reduce repeat interventions for Dupuytren contracture.

Purpose of the Study:

  • To determine the time to repeat intervention after mini-open partial fasciotomy for Dupuytren contracture.
  • To report the rate of postoperative complications associated with this procedure.

Main Methods:

  • Retrospective review of patients undergoing mini-open partial fasciotomy for pretendinous Dupuytren cord (2015-2022).
  • Preoperative and follow-up measurements of metacarpophalangeal (MP) and proximal interphalangeal (PIP) joint contractures.

  • Recording of postoperative complications, contracture recurrence (>20°), and reoperations.
  • Main Results:

    • 39 digits in 28 patients were analyzed, with a median follow-up of 5.0 years.
    • Recurrence occurred in 16 joints (36%) at a median of 3.7 years post-procedure.
    • Younger age and male sex were independently associated with contracture recurrence (P=.003 and P=.014, respectively).
    • Nine MP joints (20%) required reoperation within a median of 4.1 years.

    Conclusions:

    • Mini-open partial fasciotomy is a safe and effective treatment for pretendinous Dupuytren contracture affecting MP and PIP joints.
    • Approximately 1 in 5 joints required repeat intervention within 5 years.
    • Factors such as younger age and male sex predict recurrence, guiding patient selection and follow-up strategies.
