Implantable cardioverter-defibrillators (ICDs) can rarely cause arrhythmias. In a cardiac sarcoidosis patient, a specific timing interaction masked premature ventricular contractions, triggering ventricular tachycardia and fibrillation. Reprogramming the ICD resolved the issue.
