    Summary

    This study found lower cholinesterase (ChE) and erythrocyte cholinesterase (AChE) activity in individuals with manic-depressive illness and their relatives. These findings suggest a role for cholinergic mechanisms in bipolar disorder and related affective disorders.

    Purpose of the Study:

    • To investigate cholinesterase (ChE) and erythrocyte acetylcholinesterase (AChE) activity in patients with manic-depressive illness and their families.
    • To explore the potential role of cholinergic mechanisms in the etiology of affective disorders.

    Main Methods:

    • Measurement of plasma cholinesterase (ChE) and intact erythrocyte acetylcholinesterase (AChE) activity.
    • Comparison of enzyme activities between patients with bipolar (BP), unipolar (UP), and other affective disorders (OA), their well first-degree relatives, and unrelated healthy volunteers.
    • Assessment of enzyme activity in BP patients on lithium treatment versus those not on treatment.

    Main Results:

    • Significantly lower plasma cholinesterase (ChE) activity was observed in patients with BP, UP, OA, and well relatives from manic-depressive families.
    • Significantly lower intact erythrocyte cholinesterase (AChE) activity was found in all patient groups and their relatives.
    • Cholinesterase activities were similar in bipolar patients on lithium treatment compared to those not on lithium.

    Conclusions:

    • The study indicates a significant role for cholinergic mechanisms in the pathophysiology of manic-depressive illness.
    • Reduced cholinesterase activity may represent a potential biomarker for affective disorders and their familial predisposition.
    • Findings suggest that cholinergic system dysregulation is involved in the etiology of bipolar and related mood disorders.
