    Background:

    • Fibrinolytic activity is crucial for microcirculatory blood flow; impaired activity is linked to thrombosis.
    • Scleroderma is a condition characterized by fibrosis and vascular abnormalities.

    Purpose of the Study:

    • To evaluate cutaneous fibrinolytic activity (CFA) in patients with morphea and systemic sclerosis.
    • To assess the effect of histamine injection on CFA in these patients.
    • To determine the potential for fibrinolytic agent therapy in scleroderma.

    Main Methods:

    • Autohistographic method used to measure CFA.
    • Study included ten subjects with morphea and five with systemic sclerosis.
    • CFA was measured before and after intradermal histamine injection.

    Main Results:

    • In systemic sclerosis, CFA was normal or increased in uninvolved and involved skin, and always increased post-histamine.
    • In circumscribed scleroderma, CFA was reduced, normal, or increased.
    • Histamine injection consistently enhanced CFA in systemic sclerosis.

    Conclusions:

    • Scleroderma patients possess an endothelial plasminogen activator pool in their skin.
    • Fibrinolytic agents may be beneficial but their use should be stage-specific in scleroderma treatment.
