Summary

This study shows a two-stage 1064-nm laser treatment effectively and safely clears bruises from cosmetic injections within days. The novel protocol significantly reduces visible downtime compared to natural healing.

Background:

  • Post-procedural ecchymosis (bruising) after aesthetic injections can prolong recovery for up to two weeks, negatively impacting patient satisfaction.
  • While pulsed-dye lasers are documented for bruise clearance, data on 1064-nm picosecond/nanosecond platforms are limited.

Purpose of the Study:

  • To assess a sequential 1064-nm laser treatment protocol for accelerating the resolution of bruising following dermal filler injections.
  • To evaluate the safety and efficacy of a staged picosecond and nanosecond Nd:YAG laser approach for post-procedural ecchymosis.

Main Methods:

  • Three female patients (Fitzpatrick skin types III-IV) with facial bruises received a staged laser treatment.
  • Day 2-3: Picosecond 1064-nm Nd:YAG laser treatment (0.20-0.30 J/cm², 8-10 mm spot, 8-10 Hz, 8-10 min).
  • Day 4-7: Nanosecond Q-switched 1064-nm Nd:YAG laser treatment (2.5-3.0 J, 8 mm spot, 30-40 stacked shots) with cooling. Clinical photos and Global Aesthetic Improvement Scale (GAIS) were used for assessment.

Main Results:

  • Significant bruise fading occurred within 4-5 days, with near-complete resolution by Day 6-7.
  • All patients achieved "Much" or "Very Much Improved" GAIS scores.
  • The protocol demonstrated a 40%-60% reduction in visible downtime compared to spontaneous healing (10-14 days), with no adverse effects like pain or hyperpigmentation.

Conclusions:

  • A sequential 1064-nm picosecond-nanosecond laser regimen is a safe and effective method for rapid ecchymosis clearance post-aesthetic injections.
  • Further controlled studies are needed to optimize parameters, cost-effectiveness, and applicability to diverse skin types and other bruise etiologies.
