A rare case of left main coronary artery occlusion was successfully treated with a novel single-stent technique using minimal contrast dye. This approach offers a feasible option for complex coronary anatomy, even in resource-limited environments.

- Provisional stenting is a feasible strategy for complex quadrifurcation lesions.
- Ultra-low contrast techniques can be effective in managing coronary artery occlusions.
