Pre-implant factors like motor and genetic profiles significantly predict deep brain stimulation (DBS) effectiveness in dystonia patients. Combining these factors improves outcome prediction for personalized GPi-DBS treatment.

Background:

  • Bilateral globus pallidus pars interna deep brain stimulation (GPi-DBS) is a treatment for drug-resistant dystonia.
  • Clinical outcomes of GPi-DBS vary significantly among patients.

Purpose of the Study:

  • To explore pre-implantation factors influencing the effectiveness of GPi-DBS in dystonia patients.
  • To identify predictors of inter-patient variability in GPi-DBS outcomes.

Main Methods:

  • Collected data on genetic profiles, symptom distribution, age at onset, disease duration, and severity in 31 GPi-DBS dystonia patients.
  • Assessed motor severity using the Burke-Fahn-Marsden Dystonia-Rating-Scale (BFMDRS-M) before and after GPi-DBS.
  • Utilized multilinear regression analysis to evaluate the interplay of factors in BFMDRS-M improvement.

Main Results:

  • GPi-DBS showed significant motor improvement (47.8% at year 1, 54.3% at 5 years).
  • Lower limb, upper limb, and trunk symptoms improved more than cranial symptoms.
  • Patients with more pronounced lower limb symptoms had greater GPi-DBS effect.
  • Motor and genetic profiles predicted 73% of outcome variability; clinical profiles increased prediction to 81%.

Conclusions:

  • Motor and genetic profiles independently contribute to GPi-DBS treatment efficacy.
  • These findings support personalized prediction of DBS outcomes for dystonia patients.
