    • Functional sequelae were assessed during both awake and sleep states.

    • This case represents the second documented instance of idiopathic bilateral diaphragmatic paralysis.
    • Understanding the functional consequences is crucial for managing similar rare respiratory conditions.
    • Highlights the importance of phrenic nerve integrity for normal respiration.
