    Summary

    Plasma exchange (PE) combined with immunosuppressants shows promise for IgA nephropathy (IgAN) patients with recent acute nephritic syndrome and high IgA-containing immune complexes (IgAIC). Early treatment may lead to lasting clinical improvement.

    Background:

    • IgA nephropathy (IgAN) is a progressive autoimmune kidney disease.
    • Elevated IgA-containing immune complexes (IgAIC) are implicated in IgAN pathogenesis.

    Purpose of the Study:

    • To evaluate the efficacy of plasma exchange (PE) combined with immunosuppressive drugs in patients with progressive IgA nephropathy (PIgAGN).
    • To assess the correlation between circulating IgAIC levels and clinical outcomes following PE treatment.

    Main Methods:

    • Five patients with PIgAGN received PE and immunosuppressive therapy.
    • Circulating IgAIC levels were monitored using a conglutinin solid phase assay before, during, and after PE treatment.

    Main Results:

    • Two patients with acute nephritic syndrome, rapid progression, and high IgAIC showed significant lasting clinical improvement and decreased IgAIC levels after PE.

  • Two patients with chronic PIgAGN and slow progression showed no substantial benefit, with temporary IgAIC reduction.

    • The presence of high IgAIC levels and recent onset are indicators for potential positive response to PE treatment in IgAN.
