Perioperative chemotherapy did not improve survival for rare, chemosensitive retroperitoneal sarcomas (RPS). Further research is needed, considering potential selection bias in treatment decisions for these aggressive cancers.
Area of Science:
- Oncology
- Surgical Oncology
- Medical Oncology
Background:
- Debate exists regarding the role of perioperative chemotherapy in retroperitoneal sarcoma (RPS).
- Focus on chemosensitive subtypes: angiosarcoma, undifferentiated pleomorphic sarcoma, myxoid liposarcoma, spindle cell sarcoma, and synovial sarcoma.
Purpose of the Study:
- To evaluate the effectiveness of perioperative chemotherapy in specific chemosensitive subtypes of RPS.
- To identify factors influencing chemotherapy use and their impact on survival in localized RPS.
Main Methods:
- Population-based retrospective cohort study using the US National Cancer Database (NCDB).
- Included 851 patients with localized RPS who underwent surgery (2004-2020).
- Propensity score matching and multivariable regression analyses were employed.
Main Results:
- 26.7% of patients received perioperative chemotherapy.
- Age ≤ 60 and higher tumor grade (GIII/IV) were associated with increased chemotherapy use.
- No significant difference in overall survival was observed between chemotherapy and non-chemotherapy groups post-matching (HR 0.89, p=0.92).
Conclusions:
- Perioperative chemotherapy was not associated with improved survival in chemosensitive RPS subtypes.
- Patient age and tumor grade influenced survival outcomes.
- Selection bias is a critical consideration for interpreting these findings.
