    Summary

    Transseptal crossed vasovasostomy offers a solution for obstructive azoospermia in men with unilateral vasal obstruction and contralateral testicular damage. This procedure resulted in successful sperm retrieval and pregnancies in a significant portion of patients.

    Background:

    • Acquired obstructive azoospermia presents a complex challenge, often involving irreparable vas deferens obstruction and contralateral testicular damage.
    • Previous surgical interventions and congenital anomalies are common etiologies for vasal obstruction.
    • Testicular damage can result from various factors including torsion, mumps orchitis, and prior surgeries.

    Observation:

    • Eleven patients with acquired obstructive azoospermia underwent transseptal crossed vasovasostomy.
    • The surgical procedure demonstrated no reported morbidity.
    • Postoperative semen analyses were available for eight patients.

    Findings:

    • Four out of eight evaluated patients (50%) achieved total sperm counts ranging from 29 to 205 million.
