[Multidisciplinary care system for massive hemoptysis: current situation, challenges and prospects]

Massive hemoptysis, a critical emergency in respiratory medicine, serves as a key indicator of a healthcare institution's multidisciplinary team (MDT) collaboration capabilities and the maturity of its critical care system. Despite substantial progress in the stepwise "medication-intervention-surgery" therapeutic strategy, China still faces challenges including regional disparities in clinical capacity that compromise diagnostic and therapeutic homogeneity, persistent interdisciplinary collaboration gaps, deficient simulation-based training for specialists, and underdeveloped interventional techniques in primary hospitals. To overcome these barriers, strategic priorities include establishing a national hemoptysis quality control network to unify clinical protocols, incorporating essential airway and vascular intervention competencies into PCCM certification standards, implementing a tiered training system with enhanced technical certification (basic, advanced, specialist), and optimizing evidence-based decision pathways through real-world data analytics. By synergizing technological innovation, process standardization, and talent development, we aim to construct a comprehensive massive hemoptysis management system, achieving the transition from reliance on empirical approaches to personalized precision care in massive hemoptysis management.
