Mitral valve prolapse in myotonic dystrophy.

E W Streib, D G Meyers, S F Sun

    Summary

Patients with myotonic dystrophy (MyD) have a higher prevalence of mitral valve prolapse (MVP). This increased MVP in MyD is likely due to chest deformities, not valve structure changes, suggesting a benign, secondary form.

    Area of Science:

    • Cardiology
    • Neurology
    • Genetics

    Background:

    • Myotonic dystrophy (MyD) is a progressive multisystem disorder.
    • Mitral valve prolapse (MVP) is a common cardiac condition.
    • The relationship between MyD and MVP requires further investigation.

    Purpose of the Study:

    • To evaluate the prevalence of mitral valve prolapse (MVP) in patients with myotonic dystrophy (MyD).
    • To explore the potential causes of MVP in MyD patients, distinguishing between structural valve changes and secondary effects of thoracic deformities.

    Main Methods:

    • Prospective evaluation of 43 MyD patients and 43 age/sex-matched controls.
    • Utilized cardiac auscultation and echocardiography (ECHO) for MVP diagnosis.
    • Correlated MVP findings with physical chest appearance.

    Main Results:

    • Significantly increased MVP prevalence in MyD patients (23.3% with auscultation+ECHO, 30% with ECHO alone).
    • MVP frequency was higher in MyD patients with thin, flat chests (P < 0.007).
    • No symptoms of systemic MVP complications were observed.

    Conclusions:

    • Increased MVP in MyD is likely secondary to thoracic deformities affecting heart geometry, not primary mitral valve structural issues.
    • MVP in MyD patients is likely a benign, secondary condition.
    • Further research into the cardiac manifestations of neuromuscular disorders is warranted.
