Abstract:
Ticks are a vector of infectious diseases, both bacterial and viral. Among ticks, Lyme disease is considered to be the most common vector-borne disease in North America and Europe. Many countries in Europe are endemic for Lyme disease, especially the central and north-eastern regions. Lyme disease is characterized by numerous signs and symptoms from various organ systems including the skin, joints, heart and nervous systems. Early stage manifesting with skin symptoms occurs in most infected people. Erythema multiforme appears at the site of a tick bite usually within one month. Typical skin lesions enable the diagnosis based on the clinical picture without performing any additional tests. Diagnostic difficulties may be caused by less common atypical and multiple skin lesions, and if the patient has not noticed a tick bite. The case report concerns a child with erythema multiforme, and reviews the literature on erythema migrans and its atypical forms.
Insights
Lyme disease, a common tick-borne illness, often presents with skin symptoms like erythema multiforme. Early diagnosis is key, though atypical presentations can pose challenges.
Area of Science:
- Medical Entomology
- Infectious Diseases
- Dermatology
Background:
- Ticks transmit various bacterial and viral infections.
- Lyme disease is a prevalent tick-borne illness in North America and Europe.
- Central and northeastern Europe are particularly endemic for Lyme disease.
Observation:
- Lyme disease manifests with diverse symptoms affecting skin, joints, heart, and nervous systems.
- Early-stage Lyme disease commonly involves skin manifestations, such as erythema multiforme at the tick bite site.
- Diagnosis is often clinical, but atypical or multiple lesions, or unnoticed bites, can complicate identification.
Findings:
- Erythema multiforme is a common early skin symptom of Lyme disease.
- Typical erythema multiforme lesions facilitate clinical diagnosis.
- Atypical erythema migrans and unobserved tick bites present diagnostic challenges.
Implications:
- Recognizing typical and atypical erythema multiforme is crucial for timely Lyme disease diagnosis.
- Understanding varied skin presentations aids in managing this common tick-borne disease.
- This case report and literature review highlight diagnostic considerations for Lyme disease presentations.
