    Background:

    • A 15-year-old male presented with a history of functional systolic murmurs and radiological signs of left ventricular hypertrophy.

    Observation:

    • Angiocardiography revealed a membranous septal aneurysm, bicuspid aortic valve, and dilation of the left subclavian and left common carotid arteries.
    • Transthoracic echocardiography demonstrated mitral valve prolapse with insufficiency and a bicuspid aortic valve with minimal aortic regurgitation.

    Findings:

    • Classical TM mode echocardiography failed to detect the membranous septal aneurysm.
    • The combination of findings points to a complex congenital cardiac anomaly.
