Cross-Cultural Adaptation and Clinical Application of the Russian Version of SEPEQoL Questionnaire

Nasal septal perforation (NSP) significantly impacts quality of life (QoL), but no validated Russian-language instrument existed for comprehensive symptom assessment. This study aimed to translate, culturally adapt, and validate the SEPEQoL questionnaire for Russian-speaking populations.

Following ISPOR guidelines, the SEPEQoL was translated into Russian and administered to 30 NSP patients and 30 matched controls. Psychometric properties were evaluated, including internal consistency (Cronbach's α), test-retest reliability (intraclass correlation coefficient [ICC]), and discriminant validity. Surgical outcomes were assessed using validated SEPEQoL in patients undergoing endoscopic septal perforation repair with bilateral mucosal flaps and interposition cartilaginous graft.

The Russian SEPEQoL demonstrated excellent reliability (α = 0.985, ICC = 0.98) and strong discriminant validity (21.6 ± 6.4 vs. 4.4 ± 3.9 in controls, p < 0.01). Postoperative scores improved dramatically (Δ = 15.4 points, d = 2.23, p < 0.001), with 100% resolution of whistling (δ = 0.95) and 94% improvement in crusting. Esthetic concerns showed minimal change (δ = 0.08, p = 0.32), underscoring the need for adjunct rhinoplasty in select cases. Patient age, gender, and perforation size did not impact outcomes.

The Russian SEPEQoL is a valid, reliable tool that captures both functional and esthetic outcomes of NSP repair. Its implementation facilitates standardized outcome assessment and highlights persistent challenges in managing external nasal deformities.
