The efficacy of enhanced recovery program compared to routine care for patients undergoing knee replacement surgery:

Objectives:
This meta-analysis compared enhanced recovery program with routine care for patients undergoing knee replacement surgery.
Methods:
PubMed, Scopus, Web of Science, and the Cochrane Library, were systematically searched up to December 20, 2024, to identify randomized controlled trials comparing the effects of enhanced recovery with routine care on the outcomes of patients undergoing total knee replacement.
Results:
Eleven randomized controlled trials with 1003 patients were finally included in this meta-analysis. Compared to routine care, enhanced recovery program significantly shortened the length of hospital stay (WMD - 2.42 day, 95% CI (-4.04, -0.80), I2 = 98.98%), but did not significantly affect time to ambulation (WMD - 34.65 h, 95% CI (-70.88, 1.57), I2 = 96.51%), visual analogue scale (VAS) score (WMD - 0.02, 95% CI (-0.82, 0.77), I2 = 97.30%), and the risk of infection (RR 0.72, 95% CI (0.28, 1.88), I2 = 0.00%) and thrombosis (RR 0.83, 95% CI (0.26, 2.66), I2 = 0.00%). Although a trend of improvement was observed for all outcomes, the absence of statistical significance could be due to the low number of studies reporting each outcome. Subgroup analysis for the length of hospital stay revealed that enhanced recovery program was more effective in those with a body mass index (BMI) equal to or less than 30 Kg/m2, in individuals aged more than 65 years, and in studies with a sample size of more than 100 participants.
Conclusion:
Compared to routine care, enhanced recovery program significantly shortened the length of hospital stay.
