Background:
Social determinants of health (SDOH) significantly impact patient outcomes across medical specialties, including orthopaedic surgery. Despite growing attention to these factors, there is no clear comprehension of how composite and individual SDOH measures influence outcomes following total joint arthroplasty (TJA). As demand for TJA continues to rise, understanding these associations is necessary to improve patient outcomes and reduce overall health care costs. This review aimed to evaluate how individual and composite SDOH measures relate to TJA outcomes in the existing literature.
Methods:
Key individual SDOH metrics examined included race, income, education level, and insurance status. Composite indices such as the Area Deprivation Index and Social Vulnerability Index were reviewed to assess broader socioeconomic and environmental influences. Relevant evidence and knowledge gaps were identified through synthesis of published literature.
Results:
Existing studies demonstrate that both individual and composite SDOH measures correlate with disparities in TJA outcomes, including access to care, postoperative complications, and functional recovery. However, inconsistencies remain regarding the relative contributions of specific determinants and their interactions. Evidence also highlights a lack of standardized reporting and limited exploration of community-level interventions.
Conclusions:
Integrating both individual and composite SDOH metrics is critical to fully understand their impact on TJA outcomes. Future studies should clarify the interaction among social determinants and identify actionable interventions to mitigate disparities. Addressing SDOH within orthopaedic care can enhance patient outcomes, reduce health care costs, and help create a more equitable health care system.
