Increased 68 Ga-FAPI-04 Uptake in Cerebral Infarction

FAPI PET/CT is a promising imaging tool for the assessment of brain tumors due to very low FAPI uptake of normal brain parenchyma and a high target-to-background ratio. However, FAPI uptake has been observed in nonmalignant brain diseases. We describe 68 Ga-FAPI-04 PET/CT findings in a patient with chronic hemorrhagic occipital infarction. The infarcted area showed inhomogeneous 68 Ga-FAPI-04 uptake mimicking malignancy. This case indicates that brain infarction should be included in the differential diagnosis of FAPI-avid brain lesions.
