IntroductionCurrent guidelines advise that anaphylactic shock refractory to traditional treatments may be managed with extracorporeal life support (ECLS), but reports have been exclusive to adults.Case ReportWe present the case of a 16-year-old female with respiratory failure secondary to peanut ingestion-induced anaphylactic shock who was swiftly and successfully managed with ECLS cannulation within 3 hours of anaphylaxis onset. After less than 36 hours of ECLS, the patient was decannulated and has fully recovered from a neurologic and cardiac standpoint.DiscussionThis case highlights the importance of a multidisciplinary approach to rapid ECLS cannulation as well as the effectiveness and feasibility of ECLS in the management of anaphylactic shock in the pediatric population.
