Adult spine deformity surgery is associated with significant blood loss.
Effective perioperative blood conservation is crucial for patient outcomes.

Purpose of the Study:

To summarize evidence for perioperative blood conservation strategies in adult spine deformity surgery.
To critically analyze existing evidence and provide recommendations.

Main Methods:

A narrative literature review with systematic analysis was performed.
Evidence for various blood conservation techniques was critically evaluated.

Main Results:

Hypotensive anesthesia, tranexamic acid, surgical team composition, and surgical duration significantly impact blood loss.
Evidence for many techniques is limited, with some being poorly supported or ineffective.

Conclusions:
